Policy
All Graduate School Forms must be submitted to the graduate program assistant allowing ample time to ensure submission to the Graduate School by the posted deadline for each form.
Procedure
• These forms include the following:
• BS/MS Degree Forms (available online, except diploma card)
• Application for Admission to the Concurrent Bachelor’s/Master’s Degree Program
• Concurrent Bachelor’s/Master’s Degree Program Supplement to the Application for Admission to Candidacy
• Concurrent Bachelor’s/Master’s Degree Mid-program Review and Recommendation for Advancement to Graduate Status
• Concurrent Bachelor’s/Master’s Certificate of Completion of the Requirements for the Bachelor’s Degree
• Candidacy Application for an Advanced Degree
• Diploma card (available from graduate program assistant)
• MS Degree Forms
• Candidacy Application for an Advanced Degree (available online)
• Diploma Card (available from graduate program assistant)
• Master’s Examination or Project Report (available online)
• Application for Continuation from Master’s Program to Ph.D. (available from graduate program assistant)
• Ph.D. Degree Forms
• Candidacy Application for an Advanced Degree (available online)
• Diploma Card (available from graduate program assistant)
• Doctoral Examination Report (available online)
• Graduate School deadlines for each academic term are posted in the Calendar/Coming Events section of the Graduate school website.
