Tutorial On Reporting Maintenance Problems You May Encounter

* Burned out lights should be reported through the Facilities Management web site http://fm.colorado.edu/is/lightsout.html
* Repair work that is part of the building infrastructure (examples include water leaks, office that is too hot or too cold, broken door lock) should be reported to Rhonda Maldonado or Dan Watson (Engineering Front Desk) at 2-5071. They will work with you to obtain all the information they need to report the problem to Facilities Management Service Desk. They will include you on the email of the report. If they report the problem via phone, they will email the service order number to you for your records.
* Reports of issues in a lab should first be cleared with the lab manager (faculty member or PI is responsible for the lab), unless the problem is clearly a building infrastructure problem or is a serious life safety issue. The lab manager will determine if this is a problem that needs to be addressed by the department/center, or if it is an infrastructure problem it is to be reported to Facilities Management.
* If you encounter a life-threatening situation, such as a fire, follow the safety procedures for you lab. For such an event, activate the closest fire alarm, evacuate the area, and then inform the Engineering Front Desk (2-5071) or JoAnn Zelasko (2-7720), by cell phone or by meeting at the walkway between ECCE and ITLL. This is the response location for the Boulder Fire Department.
*If you have a recurring issue, please have all the service order information about previous reports made about the issue. Please report recurring issues to Rhonda Maldonado, and she will work with Facilities Management to address the issue. Rhonda will engage me if needed.
